ADMISSION REQUIREMENT

REQUIRED DOCUMENTS FOR APPLICATION

  • 1 passport-style photo (35 x 45 mm)
  • Printed and signed application from (original) [If a candidate is not 18 years old the application form has to be signed by a guarantor, and a copy of the guarantor’s passport must be provided]
  • B2 level language certificate (Original or Notarized copy)
  • Notarized copy of your passport or copy with original to be confirmed by University employee (pages with name, surname and expiration date)
  • High School Diploma with sworn translation* and Legislation (from the Polish Consulate or Embassy in the country where the diploma was issued) or an Apostille, with the translation (Original or Notarized copy)
  • Statement from your High School or Ministry of Education, indicating that your diploma enables you to proceed to Higher Level Education in the country where the diploma was issued with the translation (original or Notarized copy)
  • A Medical Certificate, translated* into English or Polish. You are also able to obtain this medical certificate at the medical clinic at Lazarski University. (Original)
  • Health Insurance (printout)

If you are going to study a Single Degree Program you can choose the language of the translation yourself. In case of the Double Degree Program (British degree +Polish degree) you must translate your documents into English. If you are planning on studying in the Polish language, your documents must be translated into Polish. All the translations must be conducted by a Sworn Translator or by the Polish Embassy or Consul in your country.

  • B2 level language certificate (Original or Notarized copy)
  • Notarized copy of your passport or copy with original to be confirmed by University employee (pages with name, surname and expiration date)
  • Bachelor’s Diploma with sworn translation* and Legislation (from the Polish Consulate or Embassy in the country where the diploma was issued) or an Apostille, with the translation (Original or Notarized copy)
  •  Bachelor’s Diploma Transcript with sworn translation* and legislation (from the Polish Consulate or Embassy in the country where the diploma was issued) or an Apostille, with the translation (Original or Notarized copy)
  • A Medical Certificate, translated* into English or Polish. You are also able to obtain this medical certificate at the medical clinic at Lazarski University. (Original)
